New Member Recommendations

Providing Recommendations

Instructions for SUS Member Sponsors and Department Chairs for providing Recommendations:

Recommendations will be accepted via an online form which will be emailed to you.  This allows the Membership Committee to include your important comments in the online Membership Application system.

Completing the Recommendation Form

An electronic link to the online Recommendation Form will be sent to you using the email address the candidate provided on his/her application. Once you complete the form, your response will be uploaded directly to the application.

SUS Member Sponsors may enter recommendation information directly to the online New Member Recommendation form. Alternatively, Sponsors may upload a letter of recommendation as a PDF document addressing how the applicant meets the SUS criteria for membership. Whichever option is chosen, the recommendation should fully describe the past and present activities of the applicant as they may relate to his or her future promise as a University Surgeon. Sponsors will receive membership criteria information and a copy of the applicant’s CV for reference in completing the recommendation.

Department Chair Sponsor recommendations will include an attestation the SUS membership criteria and the submitted application have been reviewed by the Chair and they affirm if the applicant meets the rigorous requirements of membership in the SUS. Department Chair Sponsors may additionally upload an optional letter of support, although this is not required. Department Chair Sponsors will receive membership criteria information and a copy of the applicant’s completed application for reference.

Please DO NOT send paper letters of support or other supporting documents. These cannot be included in the applicant’s membership packet for review, and thus will not benefit the applicant.

Recommendations will NOT be shared with the applicant, and will only be viewed by the SUS Membership Committee as part of the applicant file.

NOTE: In order to meet the eligibility requirements, SUS Member Sponsors must be members in good standing. If an SUS Sponsor is unsure of their status, they should contact Member Services at [email protected]  or call (310) 986-6442, ext 138, as soon as possible to confirm.
