SUS Membership Information


Interested persons are encouraged to review the SUS Membership Criteria Guidelines and the SUS Constitution for additional details relating to membership eligibility.

Membership Applications to the SUS may be submitted continually throughout the calendar year; however, completed applications must be submitted via the online submission site prior to the end of the application cycle: Monday, August 14, 2017 at midnight EST.  Applications and supporting documents will not be accepted after the cycle closes. Successful applicants will be reviewed by the SUS Membership Committee, and recommended candidates will be presented for approval at the SUS annual meeting, held during the 2017 Academic Surgical Congress.

- Active and Inductee Members: $325

 
- Senior Members:$260

 

SUS MEMBERSHIP APPLICATION INSTRUCTIONS

All files must be uploaded to the online submission site by the applicant in PDF format (REQUIRED). Please remember to include your name and the title of each document when naming files to be uploaded (ex: John Smith - SUS Member Sponsor Letter).

 

1. LETTERS OF RECOMMENDATION

Letters of recommendation should fully describe the past and present activities of the candidate as they may bear on his/her future promise as a University Surgeon. The Department Chair and both of the SUS Member Sponsors should be informed in advance that they are expected to provide the applicant with their letter of recommendation as only the applicant may upload supporting documents to be included with their application. Please note that hard copies or paper documents sent by fax or US Mail will not be accepted or reviewed and may result in the application status to be considered as incomplete. 
 

THREE (3) letters of recommendation are required to be uploaded for each of the following:
 

DEPARTMENT CHAIR
One (1) letter from the Chairperson of the department from which the candidate derives their appointment.


SUS MEMBER SPONSOR

Two (2) letters from the nominating SUS Member Sponsors (1 letter each from 2 SUS Sponsors).


NOTE: In order to meet the eligibility requirements, SUS Member Sponsors must be members in good standing. If an SUS Sponsor is unsure of their status, they should contact Ochun Farlice: ochun@susweb.org, in the SUS Membership Department to confirm.

 

2. CURRICULUM VITAE


Each candidate is required to upload their Curriculum Vitae.

  

3. PUBLICATIONS

The applicant must provide the five most important publications beginning with the first year of faculty appointment, as well as a minimum of 15 peer-reviewed publications, (at least 10 must be first or senior-authored) during the appointment. A lower minimum number of peer-reviewed publications will be considered by the SUS Membership Committee if published in high impact factor journals. 

 

4. GRANT FUNDING 

Please provide evidence of extramural funding of at least $50,000 per year, or total funding of at least $100,000 per year, with research independence.
 

5. EDUCATION/ADMINISTRATIVE

 

Please include details for at least one (1) of the following:


A. Departmental (or higher) teaching award.

B. Director of a resident/medical student training program.

C. Member of 2 national committees, one of which focuses on education.

D. Major leadership position held at an institution or a national level, such as: Division Chief or higher, or Director of a major, independent functional unit (i.e. SICU).

E. Active attendance and/or participation in the Academic Surgical Congress over the last 5 years.
